Job Description

SUMMARY:

Manage and supervise the daily activities of the medical case manager/care coordinator team at the New Mexico Army National Guard (NMARNG).

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Ensure that all managed Soldier care/cases progress in a timely manner to maximize retention of medically fit Soldiers IAW AR 40-501 or separate Soldiers who do not meet retention standards, retention being the primary goal.
  • Responsible for holding weekly to bi-weekly sync meetings to track 100% of all Soldier Medical, BH, and Dental Cases, report progress and help coordinate care for case resolution and closure.
  • Manage administrative actions non-clinical Case Managers/Care Coordinators.
  • Act as a site lead to ensure all CM have computer access, complete all required training, and abide by all requirements set forth within the PWS and contract.
  • Directly work with the Office of the State Surgeon (OTSS) to receive direction, goals, and objectives for the Case Managers as a whole.
  • Meet daily with the OTSS to report on readiness performance measures, matrixes and to coordinate staff functions.
  • Maintain regular and punctual attendance.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS:

  • Associate's degree in relevant area of study or Bachelor’s degree preferred. 
  • Must hold a current, active, full, and unrestricted Registered Nurses (RN) licensed in New Mexico.
  • Should have two (2) to three (3) years of relevant experience as a Registered Nurse (RN).
  • Systematic application of substantial specialized knowledge and skills derived from the biological, physical, and behavioral sciences to the care, treatment, counsel, and health teaching of individuals who are experiencing changes in the normal health processes or who require assistance in the maintenance of health and the prevention or management of illness, injury, or disability.
  • Must obtain and maintain any necessary security access and/or background checks.

DESIRED QUALIFICATIONS & SKILLS:

  • Ability to prioritize, organize, problem-solve, and coordinate daily activities to support effective improvement in individual patients.
  • Knowledge, skill, and ability to use basic office automation and information management equipment and programs (i.e., word processing, database management, email, etc.).
  • Excellent patient assessment skills.
  • Sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients and healthcare staff from diverse backgrounds.
  • Read, write, and speak English in order to communicate with all patients and other healthcare providers.
  • Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.

SUPERVISORY RESPONSIBILITIES:

Supervise and oversee the Case Managers’ assigned duties and case progression.

Job Requirements

WORK SCHEDULE:

40 hours per week. May be required to work additional hours as needed to complete assignment or project.

TRAVEL:

May be expected to travel to locations around the state asneeded during Medical Readiness Events (MRE), these eventstypically occur once a month between December and May of each year. The expected travel time is less than 25%.

None.

WORK ENVIRONMENT:

Work is performed primarily in an office setting. Employee will be exposed to a wide variety of clients in differing functions, personalities, and abilities.

PHYSICAL DEMANDS:

Position consists of sitting for long periods of time, using hands/fingers, handling, reaching with hands and arms, speaking or translating. Periods of walking, standing, bending, kneeling, stooping, crouching, and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. May be assigned occasional to moderate periods of analytical tasks requiring extensive reasoning abilities.
