Provides prompt and professional response to referrals from acute care facilities within the healthcare system assigned.
Participates in critical care and interdisciplinary rounds to collaborate and foster a pro- donation environment. Seamlessly interacts with critical care and hospital partner staff and obtains all relevant information required to make a comprehensive assessment of medical suitability for donation potential.
Delivers high quality bereavement support to potential donor families throughout the assigned hospitals and potentially in designated service area as needed.
Responsible for educating the potential donor’s authorized person(s) about the opportunity for donation and requesting authorization and/or notifying them of status of donor registry accordingly.
Obtains authorization or performs notification and conducts a Donor Risk Assessment Interview with appropriate identified individual(s).
Ensures effective communication and strong collaboration with donor families, hospital personnel, administrator on-call, physicians, related donation agencies, and relevant LiveOnNY departments.
Collaborates with hospital staff, LiveOnNY administrator on call, authorization coach, and clinical staff to determine the best time to discuss the opportunity for donation with potential donor families.
Actively participates with clinical staff huddles providing critical information related to family dynamics and identified support system.
Determines the legal Authorized Person and develops an appropriate donation conversation strategy with the healthcare partner interdisciplinary care team.
Assesses the needs of the potential donor family and provides on-going information about their loved one’s condition. Facilitates the authorization process by counseling families to help them understand brain death and the process of organ and tissue donation.
Ensures thorough and accurate completion and compliance with hospital partner standards of all paper and electronic records, including referral and donation conversation documentation in a timely manner in accordance with LiveOnNY policies.
(BA/BS) in counseling, psychology, social services, or crisis intervention is required.
2 yrs experience in organ procurement authorization with proven track record is required.
Must have excellent knowledge of the dynamics of grief and bereavement.
Experience in a trauma/critical care setting and fluency in a second language are preferred.
