Case Manager Job Description

Essential Duties & Responsibilities

• Develop a comprehensive aftercare plan for each client.
• Assist clients in developing educational and vocational goals.
• Assist clients with any legal/court issues
• Conduct and document weekly client assessments.
• Facilitate group counseling sessions as assigned by the program director.
• Manage a caseload of up to six clients and facilitate individual case manage sessions twice per week.
• Meet with clients individually at least twice a week for case management sessions and chmical dependency sessions.
• Document all group and individual sessions following DHCS and JCAHO standards.
• Assist the treatment team with any other assigned tasks

Job Requirements

• Minimum of a high school diploma or GED
• Current counseling certification through CCAPP or CADTP.
• Minimum of 2 years experience as a counselor in an AOD program.

Skill Requirements

• Strong documentation skills
• Experience in KIPU is a plus

License

• Valid driver's license (required)

Work Authorization

• First-Aid Training
• TB Test & Physical Exam
