Added Dates: April 3 -5, 9 -11, 15-19, 23-26
Hours: Monday 8a-6p Tuesday-Thursday 7:30a-5:30p Friday 7:30p-12:30p

Contract details:

• Compensation: $1200 per day
• OT: $225 /hour
• Malpractice Insurance is provided
• TRAVEL & LODGING EXPENSE is covered if necessary.

REQUIREMENTS:

• Updated Curriculum Vitae
• Professional License
• Driver’s License
• Active DEA is required
• Please ready your NPI and DEA number (DEA schedule and DEA Expiration date)
