As the DON, you'll lead our team of dedicated home health nurses on an exciting journey to redefine the way care is delivered in the comfort of patients' homes. Responsibilities:

• Lead, inspire, and empower our team of home health nurses to deliver exceptional care that exceeds expectations and enhances the well-being of our patients.
• Develop and implement innovative approaches to home health care delivery that prioritize patient-centeredness, autonomy, and holistic well-being.
• Foster a culture of compassion, creativity, and collaboration, where every team member feels valued, supported, and motivated to innovate and excel.
• Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and community resources, to develop comprehensive care plans that address the unique needs and goals of each patient.
• Provide clinical expertise and guidance to nursing staff, ensuring adherence to evidence-based practices and standards of care while promoting a culture of continuous learning and professional development.
• Oversee quality assurance and performance improvement initiatives to monitor outcomes, identify areas for enhancement, and drive continuous improvement efforts.
• Serve as a liaison between our company, patients, families, and community partners to promote positive relationships, enhance communication, and coordinate care transitions.
• Stay current with trends, developments, and advancements in home health care, and incorporate innovative approaches into our nursing practice to enhance patient outcomes and experiences.

Qualifications:

• Registered Nurse (RN) license with a minimum of 6 of clinical experience in home health care, with 4 of progressive leadership experience.
• Strong leadership skills with the ability to inspire, motivate, and mentor teams to achieve goals and exceed expectations.
• Exceptional communication and interpersonal skills, with a compassionate and empathetic approach to leadership and patient care.
• Strategic thinker with the ability to develop and execute plans to achieve organizational objectives and drive continuous improvement.
• Commitment to upholding ethical standards and promoting a culture of integrity, transparency, and respect.
