Title: RN Hospice Case Manager

Salary: $64,480 to $82,534.40 annually, DOE, plus $10,000 signing bonus with a 2-year service requirement

Description:
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our team in Moab, UT. In this role, you will identify patient and family needs and coordinate care for terminally ill patients in their homes. You will work closely with physicians, discharge planners, charge nurses, and other interdisciplinary team members to ensure the highest quality of care for our patients.

Responsibilities:

• Identify patient and family needs and develop individualized care plans.
• Coordinate care for terminally ill patients in their homes.
• Work closely with interdisciplinary team members to ensure coordinated and comprehensive care.
• Provide emotional support and education to patients and families.
• Comfortably work autonomously with patients and families in the community.
• Participate in on-call rotations as required.

Requirements:

• Current, unencumbered Utah or compact state RN license.
• Graduate of an accredited school of nursing with at least one year of recent RN experience.
• Current BLS certification.
• Valid Driver’s License and reliable transportation for patient visits.
• Willingness to reside permanently in UT.
• Ability to take 6-10 call shifts per month, with a 45-minute response time.
• Sensitivity to the needs of terminally ill patients and families.
• Ability to independently provide high standards of care in unstructured environments.
• Excellent customer service skills and ability to maintain confidentiality.
• Emotional maturity and ability to handle stressful situations calmly.
• Reliable and timely with work schedule.
• Flexibility and willingness to accept other duties as assigned.
