PACT Case Manager- Full Time
Hiring Range: $24.94/hr. - $30.55/hr.

The Care Coordinator functions as a multidisciplinary team member while demonstrating the clinical skill and experience to plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients. A Care Coordinator also provides education, consultation to families, and crisis intervention services to clients assigned to the team.

Primary Responsibilities:
  • Provide treatment coordination services for an assigned group of clients, and participate in scheduled treatment team meetings as part of the client’s Individual Treatment Team.
  • Assume primary responsibility for collaboratively developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the ITT. Use recovery-oriented language in both written and verbal communications.
  • Utilize Cognitive Behavioral approaches, Motivational Interviewing, Trauma Informed Care approaches, and stage-wise treatment strategies to intervene and motivate clients for change while providing comprehensive case management services to address needs in the following areas: safe and affordable housing, physical health and hygiene, communication with DSHS and Social Security, finances, food sufficiency, legal requirements, any requirements of an LRA, social skills, leisure and recreation, ADL’s.
  • Educate and support clients’ families and advocate for clients’ rights and preferences. Consult with families and community agencies such as DSHS, court systems, housing authorities, etc., to maintain coordination in the treatment process.
  • Participate in the PACT Comprehensive Assessment process for assigned clients.
  • Document client progress promptly to maintain a permanent record of client activity according to established policies and procedures.
  • Participate in daily staff organizational meetings and schedule treatment planning review meetings for assigned clients.
  • Participate in providing information to clients on substance use issues, such as the effect on mental and physical health and daily functioning. Support clients in planning to reduce use or abstain from the use of substances and help teach replacement behaviors.
  • Support client follow-through with activities to help clients achieve vocational or career goals.
  • Routinely identify and evaluate physical risk factors for self and others while in potentially unsafe neighborhoods, homes, around people with unknown potential for aggression, and in proximity to potentially unsafe and/or unclean structures and equipment. Coordinate with team members and participate in crisis intervention as directed by the supervisor. Consult with DCRs and law enforcement officials when appropriate.
  • Provide psychiatric rehabilitation services. Assist clients in building skills for independent living in the community, including shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation.
  • Maintain proficiency in current trends in mental health, including healthcare reform and information technology. Maintain knowledge of PACT standards and Evidence-Based Practices. Maintain a Trauma Informed Care work environment.

Minimum Qualifications
:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Entry level (no prior related work experience)
Possess and maintain a valid driver's license with an acceptable motor vehicle report and reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration.
Preferred Qualifications:
EXPERIENCE: Experienced (minimum 2 years of job-related experience)
Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
