Kitsap Mental Health Service is Hiring a FACT Case Manager Near Bremerton, WA

FACT Case Manager Hiring Range: $24.94 - $ 30.55 per hour

Additional $2.00 per hour differential

Are you passionate about making a positive impact on the lives of individuals struggling with mental health disorders? Join our team at KMHS Mental Health Services as a Care Coordinator for our FACT (Flexible Assertive Community Treatment) program. As a Care Coordinator, you will play a vital role in providing essential support and guidance to clients on their journey to recovery.

Primary Responsibilities:

  • Collaborate with the FACT team to provide comprehensive case management to assigned clients.
  • Participate in individualized treatment planning for clients, focusing on their unique needs and goals.
  • Motivate and engage clients in making positive behavioral changes to support their long-term recovery.
  • Educate clients and their families about mental health diagnoses and the recovery process for psychotic disorders.
  • Document client progress accurately and timely, showcasing the relevance of case management activities to treatment plan goals.
  • Assist clients in developing essential skills for independent living, including daily tasks and accessing community resources.
  • Contribute to crisis intervention efforts and coordinate with other team members, law enforcement, and crisis outreach teams when necessary.

Minimum Qualifications:

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.

EXPERIENCE: Entry level (no prior related work experience)

This position requires transporting clients in a personal vehicle; therefore, qualifications include successfully completion of a motor vehicle history check, possessing and maintain a current, valid driver's license in the state of Washington, and having reliable, adequately insured transportation.

LICENSURE: Agency Affiliated Counselor Registration. (Can be attained after hire)

Preferred Qualifications:

EXPERIENCE: Experienced (minimum 2 years of job-related experience)

Experience providing comprehensive case management to clients in a community mental health or social services setting.
