Housing Case Manager Job Description

Department: Housing

Reports: Operations Manager

Range of Pay:$18.00 to $21.00 Per hour

Summary
The Housing Case Manager is responsible for the intake, evaluation, coordination and provision of the services to housing participants. This position is responsible for a variety of record keeping and interpersonal tasks including the adherence to the Washington Administrative Codes (WAC) and laws pertaining to confidentiality.

Job Functions (May Include):
1. Performs in-depth intake and evaluation with housing participants, utilizing applicable tools to determine housing criteria eligibility. This includes the gathering of clinical and demographic information regarding homelessness (HIMIS info), co-occurring disorders/barriers, and behavioral patterns/changes during their residence.

2. Initiates individual case management plans with each participant outlining participants short- and long-term goals based on participant’s perception of needs and provides guided assistance in accessing applicable community resources. Acts as a liaison between critical care wrap around team members, vocational services navigator, outreach case managers, community partners and participants.
3. Consistently maintain professional boundaries with participants and coworkers.

4. Prompt, accurate completion of case notes of all interactions with participants in assigned participant files.
5. Monitors and modifies the meeting of participant goals, performs and documents intensive 1:1 case management session, and reports concerns and findings to Operations Manager
6. Generates monthly reports in relation to the meeting of project goals/ performance standards and consults with Operations Manager and CEO regarding the outcome of reports and the facilitation of any necessary performance modification.
7. Act consistently to protect the confidentiality of all participants and the integrity of professional staff communication and behavior.
8. Adhere to all of the confidentiality requirements and guidelines outlined in the 42 CFR, Part 2 federal regulations pertaining to chemical dependency treatment records.
9. Models respect and compassion to all housing participants and utilizes motivational interviewing and harm reduction principles and techniques to minimize resistance and encourage participant change.
11. Participate in Treatment Team meetings where s/he will review patient treatment progress.
12. May also be responsible for the observation of patient urine collection for the purpose of drug screen urinalysis when needed.

Qualification Requirements

Education

· High School Diploma or equivalent; preferred Associate Degree in Behavioral Health.

· Valid WA Driver’s license and vehicle with liability insurance

· Minimum 5 years with no criminal convictions

· Computer skills including Microsoft Word and Excel with the ability to learn new programs.

· Experience in the Human services and with a variety of populations (mentally ill, disable, substance abuse user etc.)

· Ability to solve problems, make decisions, and resolve conflicts and LISTEN.

· Ability to deal calmly in crisis situations.

· Strong interpersonal skills with the ability to be compassionate and firm.

· ALWAYS MAINTAIN CONFIDENTIALITY

· Knowledge of community resources

· Ability to be flexible

Responsibility

· Must have a working knowledge of the stages of change, and the principles of the housing first model.

· Must be skilled at motivational interviewing, and effectively utilize harm reduction principles and techniques.

· Must support and utilize the “vocationalizing” method of communication with all participants.

· Must have wide knowledge base of available community resources, and the criteria involved in accessing these resources.

· Must cope well with stress and not “buy into” the crises of our participants but rather, see crises as a therapeutic opportunity to assist our participants in creating change for themselves and their families.

Must have good literacy skills including the ability to document information clearly.

Additional Requirements

· Computer literate, with basic knowledge of Microsoft Office Suite, and electronic medical records systems, as well as a high level of initiative in keeping current with technological change.

· Ability to prioritize workload and activities and follow through on assigned tasks to assure completion in a timely manner.

· Ability to handle multiple tasks and switch between tasks quickly.
Dependable, able to work under pressure, receptive to change, willing to learn, cooperative approach to problem-solving

· Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic, and cultural backgrounds; good diplomatic skills, including strong conflict-resolution skills.

· Ability to present information effectively and respond to questions from participants, staff, and the general public.

Mathematical Skills and Reasoning Ability

· Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.

· Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule, or diagram format.

Physical Requirements

· The employee is regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.

· The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
