PRIMARY DUTIES / ACTIVITIES:

• Support all clinical operations as related to VBC initiatives.

• Provides seamless onboarding and integration of the external care team into the practice operations as part of the practice extension care team.

• Tracks and monitors data related to VBC performance metrics such as pay for performance incentives and quality metrics.

• Support value-based Care partnership organization’s ad-hoc requests for reports and data

• Work collaboratively with cross-functional teams within the organization to achieve set goals and objectives of the VBC programs.

• Work with provider EMR vendor to facilitate system integration between providers and VBC Partner EMR to enable EMR solutions that facilitate processes key to value-based contracting.

• Performs other duties as assigned.

EDUCATION/ QUALIFICATIONS:

• Bachelor’s Degree or equivalent.

• Minimum of 3 -5 years of experience within the healthcare industry and experience working with physician practices

• Ability to interpret and analyze medical data and information.

• Strong or advance Excel skills required.

• Proficiency in Microsoft Office Suite

EXPERIENCE:

• Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a must

• Good presentation & communication skills

• Must be productive & resourceful with the ability to meet deadlines
