Supervision: Case Managers, Case Aides, and Student Interns

Salary Range: $54,000 - $57,000/annually

Position Description:

A Case Manager Supervisor provides leadership, support, and supervision of Waiver Case Management staff members who provide case management services for individuals on Home and Community Based Waiver Service Programs in Ramsey County including: Community Access for Disability Inclusion (CADI) Waiver, Brain Injury (BI) Waiver, Community Alternative Care (CAC) Waiver and Developmental Disability (DD) Waiver. 

This position assists with employee hiring and training, intern management, planning and development of new and existing program services, reporting and contract compliance. 

This position will maintain a small Ramsey County caseload and is responsible for the delivery of client services within Keystone standards.

Essential Functions:

Staff Direction: (50 %)

  • Supervise and manage a team of Disability Services case managers 
  • Monitor staff performance and productivity and provide feedback, coaching, and discipline when needed.
  • Screen, interview, and hire case managers as needed
  • Provide regular case consultations.
  • Serve as a resource for case managers including problem solving techniques and ideas, available community resources, case management strategies and guiding participants through challenges.
  • Teach best practices and optimum customer service techniques. 
  • Conduct file reviews per program procedure and maintain caseload review spreadsheets.
  • Provide leadership and direction to disability waiver case management team on policy and procedures required to support County, State and Federal, and Keystone Compliance. 
  • Work with Program Director and Team Lead to train and orient new staff
  • Provide training for new staff and ongoing training for current staff as updates occur from Ramsey County and DHS. 
  • Organize and lead regular team meetings
  • Provide regular feedback and annual performance reviews for staff
  • Authorize time off and timesheets for staff.

Program/Contract Management: (25 %)

  • Review and assign new referrals from Ramsey County to case managers.
  • Maintain and update training materials.
  • Keep up to date on changes in programs, policies, and compliance requirements as they change through DHS and Ramsey County
  • Evaluate program requirements and work with Program Director and Team Lead to develop or maintain processes and procedures to maintain a high level of program quality and service.
  • Coordinate closely with Supervisors at Ramsey County and complete and submit required reports as necessary.
  • Review and approve requests for individual client budget amendments.
  • Prepare and maintain reports
  • Work collaboratively with internal and external partners.
  • Provide backup for staff and Program Director as needed.

Case Management: (20 %)

  • Provide person-centered case management services that assist individuals who receive Home and Community Based Waiver Services in locating and gaining access to needed waiver and other state plan services while meeting the person’s goals related to medical, social, housing, employment and various other goal areas.
  • With the individual’s involvement, develop and implement a person-centered Coordinated Services and Support Plan (CSSP) that is based on the MnChoices assessment results and within the county approved allocation for the person.
  • Evaluate, facilitate and monitor services and individual progress in relationship to established goals and objectives. Work with the participant to adjust service plans as necessary.
  • Plan and arrange for services via Service Agreement and inform Ramsey County Clerk of the proposed services to be authorized including: service type, provider, cost, frequency and total number of units needed for authorization.
  • Act as a liaison with community and referral services.
  • Maintain necessary records for continuity of service and reports to funding agencies.
  • Maintain accurate case notes. Document all actions related to case management coordination for the individual. This includes all face-to-face and indirect interactions or written correspondence with the client, client’s support system, service providers, and county staff. Documentation should reflect that active and consistent activity is taking place in an effort to support the person’s plan.
  • Complete all required paperwork, reports, and charting simultaneously and in an organized manner.

Other: (5 %)

  • Participate in team meetings, agency All Staff meetings, health plan meetings and trainings.
  • Perform other duties or projects as assigned.
  • Obtain necessary CEU’s to maintain professional licensure status if applicable.

Qualification Requirements

  • A Bachelor’s degree in Social Work or related field.
  • 2 years of Case Management experience including working knowledge of case management methods/values/principals of social work practice.
  • Understanding of professional boundaries, community resources, supervisory principles and interventions, program planning and development with familiarity with Federal and State Home and Community Based Service rules, regulations and policies and cultural competency.
  • Knowledge of case management issues for those experiencing serious mental illness or developmental disability including an understanding of mental health and social service rules, regulations and policies. 
  • Experience with billable time/units.
  • A valid Minnesota Driver’s license and insurance as travel within Ramsey County and surrounding areas for home visits is required.

Qualification Preferences

  • Social Work Licensure with the Minnesota Board of Social Work, with ability to provide licensure supervision.
  • An advanced degree in Social Work or related field.
  • Previous experience in Home and Community Based Services/Waiver programs.
  • Prior Supervisory and training experience including strong leadership and coaching skills.

Physical/Mental Requirements

Physical working conditions include frequent computer work, telephone calls and moving around the office. In addition, this position requires the employee to drive to client’s homes, meetings, etc. in the community. Current position is hybrid with remote working options in place along with required days working at the office.

Mental requirements include verbal and written communication skills, good problem solving and decision making skills, ability to easily adapt to change and capacity to handle multiple priorities. In addition, must be able to plan out and implement changes as they occur.
