Program/Department: Senior Care Management Services 

Salary Range: $46,500 - $48,000 annually 

A Hiring Bonus of $1000.00 will be awarded to the candidate selected after 30 and 90 days of employment.

Position Description:

A Disability Services Case Manager II, provides person-centered assessment, equitable support planning, facilitation and advocacy for options and services to meet the needs of elderly or adults with disabilities who are enrolled on Community Access for Disability Inclusion (CADI), Brain Injury (BI), Community Alternative Care (CAC), and Developmental Disability (DD) Waiver or Rule 185 Case Management in Ramsey County.

This position will monitor health and safety needs of individuals and intervene as necessary, ensure effectiveness of services provided and collaborate with other professionals working with those you serve.

This position offers a Hybrid work model where the selected candidate may work in the Office or Remote from home.

Essential Functions:

95% Case Management

  • Provide person-centered case management services that assist individuals who receive Home and Community Based Waiver Services in locating and gaining access to needed waiver and other state plan services while meeting the person’s goals related to medical, social, housing, employment and various other goal areas
  • With the individual’s involvement, develop and implement a person-centered Coordinated Services and Support Plan (CSSP) that is based on the MnChoices assessment results and within the county approved allocation for the person
  • Evaluate, facilitate and monitor services and individual progress in relationship to established goals and objectives. Works with the person to adjust service plans as necessary
  • Plan and arrange for services via Service Agreement and inform Ramsey County Clerk of the proposed services to be authorized including: service type, provider, cost, frequency and total number of units needed for authorization
  • Act as a liaison with community and referral services
  • Maintain necessary records for continuity of service and reports to funding agencies
  • Maintain accurate case notes. Document all actions related to case management coordination for the individual. This includes all face-to-face and indirect interactions or written correspondence with the individual, individuals support system, service providers, and county staff. Documentation should reflect that active and consistent activity is taking place in an effort to support the person’s plan
  • Meet monthly billable time requirement
  • Complete all required paperwork, reports, and charting simultaneously and in an organized manner

5% Other

  • Participate in team meetings, agency all staff meetings, county meetings and trainings.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision
  • Obtain necessary CEU’s to maintain professional licensure status
  • Perform other duties as assigned.

Qualification Requirements

  • Bachelor’s degree in Social Work, Special Education, Psychology, Human Services or related field.
  • Two years of experience working with Minnesota Home and Community based Waiver programs (Alternative Care, Elderly Waiver, CADI, DD, BI, CAC Waivers).
  • Experience working with seniors or adults/children with disabilities in a community based setting, including experience with assessing strengths and needs, and providing information, referral and follow-up to other community resources.
  • Working knowledge of case management method of social work practice. Culturally competent. Excellent written and verbal communication skills. To perform this job successfully, candidate chosen must be competent in computer skills including database and internet experience as well as ability to type proficiently.
  • Strong organizational skills, attention to detail, flexibility, teamwork and the ability to manage multiple demands each day. Ability to work independently while using supervision appropriately.
  • A valid Minnesota driver’s license and current insurance as travel within Ramsey County and surrounding areas for home visits is required.
  • Must be able to pass a background and motor vehicle check.

Qualification Preferences

  • Minnesota Social Work Licensure preferred.
  • Two years of Social Worker experience working with Medicaid Waiver programs in Minnesota or working with Managed Care Organization providing Minnesota Senior Health Options (MSHO) or Minnesota Care Options Plus (MSC ).

Physical/Mental Requirements

  • Physical working conditions include frequent computer work and telephone calls and moving around the office. In addition, this position requires the employee to drive to meet with clients in their homes on a daily basis.
  • Mental requirements include verbal and written communication abilities to conduct client assessments and develop care plans. Must possess good people and listening skills and be able to maintain client confidentiality. Must be able to professionally interact with Keystone staff and management, clients in the community and other social service organizations.
  • Equipment/technology requirements include familiarity with Microsoft Office software, copier and fax machine.
