Kendal at Home has an incredible opportunity with a comprehensive training program for a Geriatric Care Manager / Care Coordinator to join our dynamic team. The Care Coordinator plays a crucial role in ensuring the well-being and safety of our Members as they age in the comfort of their homes. This position involves conducting assessments, developing personalized care plans, and coordinating resources to meet the unique needs of each Member. Our Care Coordinators also play a pivotal role in marketing our program and actively engaging in the sales process. The ideal candidate will have a passion for serving older adults, possess strong organizational and communication skills, and demonstrate a commitment to maintaining the highest standards of care.

Essential Functions

• Manage a caseload of members with varying acuity levels through communication with members, families, and support resources.
• Conduct assessments as necessary and oversee and evaluate care plans to support changing needs.
• Identify and provide education and resources to support members ability to age in place.
• Demonstrate an in-depth understanding of the Senior Living market and articulate Kendal at Home’s distinctive value proposition within this sector.
• Exhibit strong public speaking skills and the ability to engage effectively with the prospective member population, conveying information in a compelling and persuasive manner.

Job Type: Full-time

Pay: $48,000.00 - $65,000.00 per year
