POSITION SUMMARY

Peer Educators encourage engagement in care and support adherence to treatment by providing client centered individual and group level skill building activities. Peer Educators are integral to the Treatment Adherence Program and provide specialized services in a professional environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide the following interventions: individual, group, short-term, and just-in-time that support a client’s increased knowledge of HIV self-management and adherence to treatment, offering hope and living proof that living with HIV is possible.
  • Deliver individual client educational components (e.g., HIV 101, Lab Values, Adherence) and support patients in navigating the health system and community resources.
  • Accept client referrals from Manager, Ryan White Case Managers, collaborative partners via email and SCOUT and maintain a caseload including clients Lost to Care and at risk of being Lost to Care.
  • Contact clients in a timely manner to provide emotional and educational support, and encourage self-advocacy to help clients with continued engagement in care.
  • Participate in Multidisciplinary meetings and with other community-based organizations (e.g., hospitals, clinics, or private offices) as requested.
  • Identify and develop client directed treatment plan goals and monitor ongoing achievement of goals; assess barriers and provide support to schedule medical or case management appointments to engage or re-engage in care.
  • Work collaboratively with HIV Primary Care, Behavioral Health and Case Management teams to identify support and engage clients including Lost to Care clients who can benefit from Peer services.
  • Conduct Multidisciplinary Client Case Conferences when appropriate with the healthcare team (i.e., Case Management, Linkage to Care Coordinators, and Primary Care etc.).
  • Document group and individual encounters in SCOUT to maintain accurate records of client Peer sessions/group meetings.

MINIMUM REQUIREMENTS

  • High school diploma or equivalent
  • One year of experience or completion of a leadership training program.
  • Experience using computers, including Microsoft Office and email communication.

PREFERRED REQUIREMENTS

  • One year of experience working in the healthcare field.

Working Conditions

General working conditions are in a healthcare facility. As such, there is a potential exposure to virus, disease, and infection from patients while performing the duties of this job. May experience traumatic situations including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.

Physical Demands

While performing duties of this job, employees are regularly required to set, walk, and stand; talk and hear; both in person and by telephone; use hands repetitively to finger, handle, feel, or operate standard office or clinical equipment; reach with hands and arms; and occasionally lift up to 20 pounds.

Mental Demands

While performing the duties of this job, employees are regularly required to use written and oral communication skills; read, analyze, and interpret data, information and documents; analyze and solve non-routine and complex problems; use math and mathematical reasoning; observe and interpret situations; learn and apply new information or skills; perform highly detailed work on multiple, concurrent tasks; work under intensive deadlines with frequent interruptions; and interact with others outside of their department.
