Patient Service & Discharge Coordinator Position Summary:
Coordinates the day-to-day activities of a multi-faceted medical and/or surgical practice to ensure a smoothly functioning office and good patient relations
Resolves patient problems, third party issues, or refers patients to appropriate resources
Coordinates the post discharge appointment process for inpatients on assigned unit(s)
Works with multidisciplinary care team prior to discharge to determine appointments that are needed for each patient post discharge
Serves as resource in team environment and works extensively with Housestaff
Responsible for assisting members of the care team with record retrieval and other administrative tasks such as phone calls, faxing, etc
Offers friendly, courteous, and confidential assistance to patients to ensure that the patient has a positive discharge experience
Provide patients with information on physicians, directions to locations and educational materials
Answer patients or physicians’ questions.
Education and Work Requirement:
A.A. degree or equivalent required
B.A. degree preferred
Experience beyond the required work experience requirement may be substituted for the degree on a two (2) years of experience for one (1) year of education basis
Requires a minimum of five (5) years related experience in medical office, hospital, medical call center, or similar customer service environment
