The Forensic Case Manager position is responsible for providing a wide array of culturally sensitive, recovery oriented, trauma sensitive, direct clinical services provided in conjunction with other staff. Services provided by this position may include assessment, treatment planning, referrals, and case management. Master's degree is preferred, and preference will be given to those holding a master's degree.
Case Manager's primary duties include assisting consumers in their rehabilitation efforts, whether in the community, or in their return from a Department of Corrections (DOC) institution. The Case Manager will provide medium intensity case management services for consumers with mental health and/or substance use disorders, providing and coordinating the team’s clinical approach for consumers on their caseload, and supporting and sharing duties with team members. Case management activities include but are not limited to: Psychotherapy, AODA services, activities of daily living training, vocational endeavors, working on finances and budgeting, obtaining and maintaining housing, assistance with legal problems and obligations. family systems counseling, obtaining and coordinating health care services. Department of Health Services (DHS) contract requirements for implementation of Motivational Interviewing (MI):
The Case Manager will also complete timely and professional documentation. The Case Manager reports directly to the assigned supervisor with overall program supervision provided by the Team Leader. Minimum annual service hours are 1352.
