Job Overview
The Occupational Therapist will complete screening,evaluations, re-evaluations, and discharges as supervised by thePhysician. The Occupational Therapistwill provide supervision to Occupational Therapy Assistants as directed by theDirector of Rehabilitation. PER-DIEM WEEKENDS
Responsibilities andDuties:
• Evaluates patients assigned per MDorders and as indicated, and establishes appropriate Plan of Care and Goalsthat meet the specific needs of the individual.
• Re-assesses patients and completesUpdated Plan of Care least every 30 days, or as indicated on initialevaluation, in order to determine need for continued services ordiscontinuation of services based on patient progress. Carry out treatment planas indicated.
• Screens all patients in facility ona quarterly basis, to identify those who have experienced declines in theirfunctional abilities and determine if Occupational Therapy intervention isindicated.
• Provides thorough dischargeplanning that will allow the patient to function at a maximal level of independencein the least restrictive environment.
• Observes and records patient’sprogress, response to treatment and limitations and maintains this informationin electronic medical record.
• Completesrequired documentation at point of service.
• SupervisesOccupational Therapy Assistants as assigned by Director of Rehabilitation.
• Identifies needs for programdevelopment and implements programs according to company policies andprocedures.
• Coordinates in-services forfacility staff as need is determined, to ensure safe and effective carry overof Restorative Programs and overall call of the residents.
• Attends and reports accurately apatient’s status at weekly UR meeting and interdisciplinary care plan meeting.
Qualifications:
• Education:Graduate of an accredited therapy program
• Licenses/Certifications:Current Occupational Therapist New Jersey state license
• NBCOTCertified
At least 5 years of LTC experience required
