The Occupational Therapy Assistant will implement the planof care as established by the Occupational Therapist in a professional andcompetent manner. COVID Vaccine required.

Responsibilities andDuties:

• Works under the direction ofOccupational Therapist to plan, implement, or administer programs that restoreor enhance performance in individuals with functional impairments.

• Assists in instructing patients andfamilies in basic living skills, the care and use of adaptive equipment, andhome programs.

• Assistsoccupational therapists with implementing treatment plans.

• Reportsto supervisors on patients' progress and discharge recommendations

• Attends and reports accurately apatient’s status at weekly UR meeting and interdisciplinary care plan meeting.

• Observes and records patient’sprogress, response to treatment and limitations and maintains this informationin electronic medical record.

• Completesrequired documentation at point of service.

• Providestraining and education to families/caregivers.

• Screens all residents in facilityon a quarterly basis, to identify those who have experienced declines in theirfunctional abilities and determine if Occupational Therapy intervention isindicated.

• Identifiesneed for modification of treatment plan and reports to the RegisteredOccupational Therapist.

• Identifies needs for programdevelopment and implements programs according to company policies andprocedures.

• Coordinates in-services forfacility staff as need is determined, to ensure safe and effective carry overof Restorative Programs and overall call of the residents.

Qualifications:

• Education:Graduate of an accredited therapy program

• Licenses/Certifications:Current Occupational Therapy Assistant New Jersey state license

• NBCOTCertified

• CPRCertified
