General Responsibilities

The Program Coordinator, Bronx Assertive Community Treatment (ACT) Older Adult Services, is responsible for managing medical records, supporting daily staff communication by assisting in maintaining documentation processes in a central location for access and visualization by all team members, maintaining CAIRS documentation, maintaining accounting and budget records for individual and program expenditures, utilizing resources to help inform team members (PSYCKES, Regional Health Information Organization (RHIO), etc..) and performing reception activities (e.g. triaging calls and coordination communication between the program and individuals).

Primary Responsibilities

  • Manage the ACT program office, handling calls, triaging, and directing urgent matters to clinical staff.
  • Maintain and coordinate staff and client schedules based on Team Leader directives and team meeting outcomes.
  • Prepare and manage documentation such as letters, memos, and reports, including data entry and reporting
  • Update and maintain ACT team literature, client handouts, and marketing materials.
  • Participate in daily organizational meetings and ensure office supplies and forms are adequately stocked.
  • Process and prepare documents for electronic record keeping and conduct reminder calls for appointments.
  • Assist clinical staff with electronic systems, monitor client billing processes, and perform other assigned duties.
  • Engage in service plan development, staff meetings, training sessions, and assessment processes.
  • Provide crisis intervention support, including on-call services, and maintain progress notes and relevant data for ACT services.
  • Address client needs in various areas such as problem-solving, wellness, housing, education, social supports, and primary care.
  • Additional tasks as assigned

QUALIFICATIONS:

  • Minimum education: High school diploma, with preference for an Associate or Bachelor’s degree in human services, business management, or related fields.
  • Proficiency in typing and Google Workshop
  • Capabilities in online reporting, documentation, referrals, research, and data management.
  • Ability to provide culturally competent services and effectively collaborate with clients’ support networks.
  • Willing and able to work a flexible schedule which may include weekends and holidays.
