DETAILS

General Responsibilities

The Nurse Practitioner, Bronx Assertive Community Treatment (ACT) Older Adult Services, in conjunction with the Associate Director, has overall clinical responsibility for monitoring individual treatment and staff delivery of clinical services. The Nurse Practitioner provides both psychiatric and medical assessment, along with treatment. The Nurse Practitioner will also provide clinical supervision, education and training to the ACT Team, provide psychopharmacology services related to the development, maintenance and supervision of medications for psychiatric and medical purposes.

Primary Responsibilities

  • Conduct substance use and psychiatric assessments, including substance use/psychiatric history, course of illness, response to treatment, mental status examination, and DSM-V diagnoses.
  • Present and discuss assessment/client care at team meetings and case conferences.
  • Prescribe psychotropic medications and conducting regular assessments of therapeutic responses and side effects, including education of clients regarding psychotropic medications.
  • If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR.
  • Assess and discuss with the client's eligibility for MAT.
  • Provide recommendations on level of care and treatment setting to meet client’s individual needs.
  • Receive and review toxicology and lab results, incorporating into electronic health record.
  • Coordinate care to address psychiatric/medical issues in collaboration with other staff.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Collaborate and coordinate with outside care providers and referral sources, as needed.
  • Provide support, education, and counseling to family members/significant others of clients, engaging them in the treatment/recovery process, to assist in their family member’s treatment and recovery.
  • Provide on-site crisis assessment and management of clients both on-site and over the phone.
  • Additional tasks as assigned

Qualifications

  • Valid New York state license for practice as a Psychiatric Nurse Practitioner;
  • Possess a current DEA Registration Number.
  • Possess a NPI number.
  • Possessa Medicaid/Medicare Provider Number.
  • Good writing/record keeping skills.
  • Demonstrated organizational skills.
  • Excellent writing/record keeping skills.
  • Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
  • Computer-literacy, with ability to learn program-specific software. Should be conversant with current MicroSoft and Google applications.
  • Strong commitment to JASA’s mission
  • Willing and able to work a flexible schedule which may include weekends and holidays.
  • Ability to travel using mass transit
  • Experience working with people with serious mental illness, and co-occurring substance use disorders, and primary health care problems, preferred
