Job Description:
Job Title: RN Care Manager
Responsibilities:
- Conduct face-to-face needs assessments and telephonic follow-ups with members at various locations, adhering to state and national guidelines.
- Coordinate care for members with chronic health issues across different service areas, ensuring safety and cost-effectiveness.
- Educate members and caregivers on available healthcare resources and services, facilitating goal achievement.
- Collaborate with healthcare teams to develop personalized care plans and ensure appropriate service delivery.
- Document all case management activities in compliance with privacy regulations and company policies, while also assisting members in overcoming care obstacles.
Qualification:
- Active RN License issued by DE State
- 5 years of Intensive Case Management and Discharge Planning experience
- Experience with ordering DME Equipment
- Experience completing Assessments, developing Service Plans and Care Plans
- Experience collaborating with PCP’s, Occupational Therapists, Behavioral Health, and Providers
Job Type: Contract
Pay: $40.00 - $44.00 per hour
Expected hours: 40 per week
