Home Health Registered Nurse (RN) Case Manager

 Opportunities in: Boulder, Niwot, Louisville, Superior, and Surrounding Areas

Home Health Registered Nurse (RN)

 Opportunities in: Fort Collins, Berthoud, Loveland, Greeley, Firestone, Fredrick, Brighton, Dacono, Boulder, Lyons, Mead, Platteville, Thornton, Northglenn, Arvada.

Compensation: $50-$150 per visit (dependent on visit type), PLUS mileage and travel compensation with on-call requirement 

Status: Full or PRN

On average, our full time RN's make between $75,000 and $85,000 a year

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

  • Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
  • Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
  • Examine patients, do assessments, document progress and report changes to their physician
  • Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
  • Coach family members on the patient’s plan of care, medication and home safety
  • Ensure goals are met and coordinate discharge from services

A few must-haves for Home Health Registered Nurses:

  • Active Registered Nurse (RN) License in Colorado
  • Graduate of an accredited school or program of nursing
  • At least 1200 hours of experience as an RN 
  • Health care CPR/BLS Certified (demonstration course required)
  • Home health care experience preferred
  • Great organizational skills.
  • Able to manage patients with chronic illnesses
  • Pass a background check and drug test (we are required to test for THC)
  • No Covid Vaccination Required
