Interfaith Community Se is Hiring an Outreach Peer Support Specialist Near Escondido, CA

Please note work hours: Thursday thru Monday, 10:00am to 6:30pm OR Friday thru Monday, 9:00am-8:00pm

JOB SUMMARY 

The Outreach Peer Support Specialist (OPSS) is a field-based position that will provide culturally competent outreach and engagement services to build relationships with individuals experiencing homelessness. The OPSS uses a person-centered, compassionate approach to resolve homelessness through the offer of services tailored to the specific needs of each individual. The OPSS will provide linkage to services to address the primary causes of homelessness such as SUD treatment, mental health treatment, and housing resources. The OPSS will provide ongoing support and follow up services throughout the individuals course of treatment in order to ensure active engagement.

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Provides support by drawing on own experiences and professional skills to build rapport and trust with clients.
  • Works with Homeless Outreach Teams to establish initial connection with persons experiencing homelessness and provide linkages to available resources
  • Continues to maintain contact and build relationship with individuals in precontemplation stage of change.
  • Provides education on overdose prevention and reversal.
  • Participates in harm reduction activities in partnership with Neighborhood Healthcare Harm Reduction program and Outreach Teams.
  • Conducts Community Assessment Tool (CAT) and enters data into CES/HMIS;
  • Provides services for timely entry into Detox/Residential treatment including connection to health care provider for medical clearance and Medication Assisted Treatment services.
  • Assists individual in navigating enrollments into available community resources 
  • Develops and maintains effective relationships with SUD, mental and medical health providers
  • Maintains accurate records and files and prepare reports as required
  • Monitors compliance with grant and contract requirements
  • Ensures the confidentiality of client information and health records
  • Transports clients in safe and timely manner while observing traffic laws and regulations 

EXPERIENCE / EDUCATION 

  • High school diploma or equivalent required 
  • 1 years of experience with the process of recovery from mental illness or substance use disorder, either as a consumer or as the parent, caregiver or family member of a consumer
  • Must enroll in Peer Support Specialist Certification program within 60 days of employment
  • Must possess and maintain a valid driver’s license and have a clean driving record for insurability
