Interfaith Community Se is Hiring a Housing Stability Case Manager Near Escondido, CA

JOB SUMMARY

This position will provide Housing Stability case management services focused on ending homelessness for homeless individuals enrolled in the Recovery and Wellness Center. The Housing Stability Case Manager will provide direct services to include: linkage to housing resources including Coordinated Entry System, co-creating a client-driven housing plan, housing identification services and support, case conferencing, budgeting, and client advocacy (resources & referrals). 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Provides direct services to homeless RWC program participants including eligibility screening and assessments, housing stability case management, and group facilitation on topics such as credit counseling, budgeting, employment and other common barriers to maintaining permanent housing.
  • Ensures clients are assessed, entered, and updated in the Coordinated Entry System/HMIS.
  • Assists program participants in developing a housing focused plan that promotes permanent housing and self-sufficiency; identifies participant strengths and barriers to stability and assists participants in reducing barriers and linking to resources and services.
  • Identifies and maintains current information on internal and external housing stability resources and advocates for program participants. 
  • Collects all necessary documentation needed to support assistance provided; documents services and enters data per contract guidelines and agency policy. 
  • Provides information and warm referrals to appropriate social service agencies and/or community programs.
  • Assists in development of and encourage adherence to a personal budget; provides budget counseling and education.
  • Works and collaborates with volunteers, interns and other agency personnel; serves as a resource to other RWC counselors on housing-related supports.
  • Assists homeless program participants in locating and securing housing of their choice, including matching through the Coordinated Entry System. 
  • Participates in care coordination meetings, staff meetings, and other required meetings. 
  • Works to ensure appropriate housing resources are in place at program discharge, provide support during level of care transitions, and provide warm-handoffs, as appropriate, to ensure continuity of housing supports.
  • Conducts follow-up with participants at designated intervals, per contract guidelines and agency policy. 
  • Performs additional duties as assigned by supervisor.

EXPERIENCE / EDUCATION

  • Bachelor’s degree in Social Work, Social Services or related field required, Masters preferred
  • 1 years of relevant work experience required
  • Experience working with individuals experiencing homelessness or SUD preferred
  • Experience providing case management preferred
