Description
Job Summary: The Oncology Care Coordinator plays a vital role in providing comprehensive support and coordination for patients undergoing oncology treatment. They serve as a liaison between healthcare providers, patients, and their families, ensuring seamless communication and effective care delivery throughout the treatment process. This position requires a deep understanding of oncology care protocols, excellent organizational skills, and strong interpersonal abilities to assist patients in navigating their treatment journey.
Responsibilities:
1. Coordinate Care: Facilitate communication between oncologists, nurses, and other healthcare professionals involved in the patient's treatment plan to ensure continuity of care.
2. Resource Referral: Connect patients and their families with support services, such as counseling, financial assistance, transportation, and community resources (such as support groups to enhance their overall well-being.
3. Appointment Coordination: Schedule appointments for medical tests, consultations, and treatments, ensuring timely access to healthcare services and adherence to the treatment plan.
4. Documentation and Record Keeping: Maintain accurate and up-to-date records of patient interactions, treatment plans, and progress notes in compliance with regulatory standards.
5. Emotional Support: Offer emotional support and encouragement to patients and their families, addressing their fears and anxieties associated with cancer diagnosis and treatment.
6. Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the delivery of oncology care, identifying areas for improvement, and implementing best practices.
7. Collaboration: Collaborate with multidisciplinary teams, including physicians, nurses and navigators, social workers, and case managers, to optimize patient outcomes and streamline care delivery processes.
Requirements
· Education: Bachelor's degree in nursing, social work, healthcare administration, or a related field. A master’s degree is preferred.
· Licensure: Valid licensure or certification as required by the state, such as Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Certified Oncology Nurse (CON).
· Prior experience (preferably at least two years) in oncology care coordination, case management, or healthcare navigation is preferred.
· Knowledge: Comprehensive understanding of oncology treatment modalities, disease processes, and supportive care services. Familiarity with healthcare regulations and compliance standards.
· Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare professionals.
