Integrated Behavioral Health Services is Hiring a Community Support Worker Near Lincoln, NE

Roles and responsibilities

  • Complete case management assessment & intakes on all clients entering caseload as assigned.

  • Develop appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients.

  • Monitor, evaluate, and record client progress with respect to relevant treatment goals in an accurate and timely manner. 

  • Provide on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan to help the clients to achieve the stated goals and objectives.

  • Provide face-to-face reviews with the client and/or relevant staff on a regular basis the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirements.

  • Document all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintain the case records in accordance with agency and regulatory standards and requirements.

  • Participate in interagency planning and service coordination activities as directed to improve and enhance service continuity and effectiveness for clients.

  • Maintain close communication with the medication provider for input regarding medication compliance, side effects of medication, and medication changes and alerts these members of the staff of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention.

  • Maintain service logs and turn them in on a weekly basis so that staff and client service information can be reviewed by supervisors and management.

  • Use electronic and manual systems to document progress reports, log notes, letters, faxes and phone calls; confirm coding of services rendered is ethically appropriate and results in successful billing. 

  • Input, organize and maintain detailed clinical performance data to comply with applicable government or program requirements.

  • Demonstrate fiscal responsibility by supporting agency billing policies and procedures.

  • Meet guidelines and program requirements for fidelity and evidence-based programming; ensure professional performance effectively.

  • Attend and/or participate in program meetings and agency All-Staff meetings.

  • Complete all initial and ongoing required trainings and certifications.

  • Perform other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.

  • Manage special projects as assigned by Program Director.

  • Perform other duties as assigned by the Executive Director and/or Owner.

Qualifications and Education Requirements

  • Bachelor’s Degree in Human Services area from an accredited college or university, 2 years relevant work experience, or 2 years lived experience.

  • Valid Nebraska Driver’s License with a driving record that meets insurability requirements 

  • Experience with State of Nebraska Medicaid Standards to include fraud, waste, and abuse standards.

  • Experience and confidence identifying and implementing evidence-based practices.

  • Ability to work flexible hours. 

  • Strong written and oral communication skills.

  • Proficiency in computer skills, especially using Outlook, Word, Excel and customized databases. 

  • Ability to work in and adapt to the culture of a multi-cultural, community-based organization. 

  • Commitment to maintain confidentiality. 

  • Understanding of the Agency’s mission and culture and the ability to maintain appropriate boundaries with staff and clients in all circumstances.

  • Employment contingent upon successful completion background checks.

  • Good interpersonal skills, ability to relate well with persons from a variety socioeconomic and cultural groups. 

  • Ability to lift supplies up to 25 lbs. to waist height. 

  • Reliable transportation, valid Nebraska Driver’s License with a clear driving record and proof of insurance.

Measurables 

  • Average 80 units per week or 320 units per month.

  • Complete progress notes within 24 hours of service.

  • Print and file progress notes and collateral contact notes within 7 days of service.

  • Submit billings Wednesdays by 12 pm (or next business day if Wednesday is holiday).

  • Complete initial and ongoing annual agency forms.

  • Complete DLA-20’s every 90 days.

  • Turn in DLA-20 scores for outcomes/data collection.

  • Complete treatment plans at minimum every 90 days.

  • Complete authorizations/reauthorizations one month prior to due date or by the 1st of the month in which they are due.

  • Attend weekly program meetings.

  • Attend agency All Staff meetings.

  • Staff all cases on caseload a minimum of every 30 days.

  • Complete all initial and annual trainings including DLA-20, CPR and First Aid, CPI, and AccreditationNow trainings.

  • Complete incident reports when applicable within 24 hours of critical incident.

  • Follows IBHS discharge policy. 

  • Oversees rehabilitation, recovery, and treatment plan for each individual client in a timely manner per Medicaid, CARF, and best practice standards.

  • Actively coordinates care of all clients with other IBHS programs/staff, primary care physician, medication provider, therapist, family, and other supports and individuals on the rehabilitation team as applicable.

  • Follows IBHS Business Operating System.

  • Accountable to job description.
