Lutheran Family Service is Hiring a Remote Community Health Worker
- Have a passion for and experience working with individuals and families to make sure they have the knowledge, support, and resources needed to meet their social and health needs.
- Have experience successfully creating client or patient-centered action plans with community members and connecting them to services and resources from local nonprofits and social service organizations.
- Have a deep understanding of how to navigate barriers that individuals face when attempting to access community-based services or support.

- The Community Health Worker (CHW) plays a key role in supporting the people we serve who have high social determent of health needs in screening, assessment, and care navigation. The individuals served will be assigned to the CHW by one of our insurance carriers.
- Provide care coordination and resource navigation to an assigned caseload of community member clients with unmet social needs.
- Conduct consistent intake, follow-up, and coaching to clients to assist with enrollment in community services/benefits/programs for which they are eligible.
- Administer social determinants of health (SDOH) screening, intake forms, and assessments.
- Assist clients with prioritizing goals and creating client-centered care plans.
- Coordinate with community organizations and resources to help clients meet their needs.
- Provide resources to clients to improve their health literacy and self-sufficiency.
- Take a proactive approach to assist with assigned cases (e.g. help schedule appointments, complete applications, make reminder calls, etc.)
- Maintain client privacy and uphold confidentiality at all times.
- Demonstrate commitment in adhering to the Path Assist program’s prescribed model.
- Participate in weekly team meetings, workshops, and training to expand knowledge of department priorities, while remaining current on new developments, as required.
- Ability to commute to and from client’s homes and may transport clients for appointments as approved.

- High school diploma or GED is required.
- Minimum of two years relevant work experience providing direct care coordination services to individuals and/or families.
- Experience working with nonprofits, social service providers, faith-based groups, and/or government agencies that address social determinants of health.
- Demonstrated understanding of and ability to work with people of diverse backgrounds.
- Demonstrated ability to work with individuals regarding a variety of behavioral health needs.
- Knowledge of community services organizations and related human services.
- Bilingual language skills preferred (English and another language(s) spoken by our client populations which may include Spanish, Arabic, Kurdish, etc.).
- Valid driver’s license, auto insurance, and ability to drive personal vehicle for agency work.
- Prolonged periods of sitting at a desk and working on a computer.
- Hybrid work environment (Health 360 office in Lincoln, field environment, and remote work).
- Company-issued laptop and cell phone.
- Travel to various locations to support client services, as needed / as applicable by region.
- Must be able to work a flexible schedule, including evenings and/or weekends (as needed).
- Occasional lifting of office items no more than fifty (50) pounds.
