Registered Nurse Team Manager

Overview:
We are seeking a highly skilled and experienced Registered Nurse Team Manager to join our healthcare team. The Team Manager is the leader of the Patient Care Team and that member of the team whose function is to: supervise, evaluate and coordinate the various component members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement; serve as patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the team’s patients; assume responsibility for the maintenance of patient records from intake through discharge or transfer to bereavement.

Skills:
- Bachelor of Science in Nursing (RN license)
- Minimum of 1 year of experience in nursing management or a related role
- Minimum of 1 year of experience in homecare
- Excellent communication skills, both verbal and written
- Ability to multitask, prioritize responsibilities, and make critical decisions
- Strong problem-solving skills and attention to detail

Pay: $100,000.00 - $105,000.00 per year
