INFOPRO LEARNING PRIVATE LIMITED is Hiring a RN Case Manager Near Boston, MA
RN Case Manager opening in Boston, MA.
This is a 13 weeks contract with possibility of extension
Schedule: 40 hours a week, Mon-Fri, 8 hours between 7.30am-5.30pm
Hire can move to 4x10s after the first week of orientation
Essential duties:
- Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans
- Collaborate with interdisciplinary teams to coordinate and manage patient care across various healthcare settings
- Utilize utilization management techniques to ensure appropriate utilization of healthcare resources
- Monitor patient progress, evaluate outcomes, and make necessary adjustments to the care plan
- Provide patient education on disease management, treatment options, and preventive measures
- Document all patient interactions, interventions, and outcomes accurately and timely
- Adhere to HIPAA regulations and maintain strict confidentiality of patient information
- Collaborate with primary care physicians, specialists, and other healthcare providers to ensure continuity of care
- Assist in discharge planning and facilitate transitions of care for patients.
- Provide support and guidance to patients and their families throughout the healthcare journey
Requirements:
- A current Massachusetts Nursing license
- Recent experience
- Covid vaccinations (two doses and first booster)
