Position Summary:

The LPN will provide skilled nursing care under the direct supervision of the Registered Nurse. The LPN will be responsible to perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Join us and help us grow our newest Agency.

Essential Functions: 

1. Assist the Registered Nurse in carrying out the plan of care.
2. Assist the Registered Nurse in performing specialized procedures, as directed, by the Registered Nurse under physician's orders.
3. Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
4. Recognize, and understand, the effects of social and economic problems upon patients and provides for the emotional and physical comfort and safety of patients.
5. Help teach the patient the appropriate self-care techniques.
6. Foster cooperative effort, among personnel, by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
7. Assume responsibility for personal and vocational growth and development, through active participation in nursing organizations and participating in in-service programs and other on the job learning.
8. Prepare clinical and progress notes (not to include admission).
9. Observe, record, and report, to the appropriate person, patient symptoms, reactions, and changes 
10. Assist with the rehabilitation of patients, according to the medical care plan, by encouraging the interests and special aptitudes of the patient and encouraging patients to help themselves using their own capabilities.
11. Further assist, in rehabilitation, by reporting any abnormality in the patient’s range of motion, body mechanics, or body alignment. 
12. Actively participate in nursing organizations; promote, and participate in, in-service programs.
13. Must be licensed by the State of Florida as a Licensed Practical Nurse.
14. At least 1-year previous experience in community/home health agency.
15. Must have criminal background check.

Must have CPR certification. Online certification is not accepted.

Education: 

Graduated from an accredited School of Practical Nursing.

Experience: 

1. One, or more, years in Home Health Agency or in a hospital setting. Home health experience preferred.
2. Proficient with Microsoft Office suite.
3.  Strong verbal and written communication skills, including ability to conduct effective presentations.
4. Critical thinking, sound decision-making and solid leadership ability.
