Medical Records Director Position: Job Duties, Benefits and Qualifications

Reporting to the Administrator, the Medical Records Director is responsible for maintaining, initiating, and filing resident records in the facility. Here are the specific job duties, benefits, and qualifications for the role.

*Job Duties:**

  1. Maintain and file resident records in the facility.
  2. Ensure the implementation of the current policy and procedures for the medical records department.
  3. Monitor the department budget as directed.
  4. Participate in the facility Quality Assurance and Assessment Committee.
  5. Maintain a quantitative/qualitative audit/monitoring process.
  6. Conduct comprehensive audits of charts on the following:
    1. Admission
    2. Advance Directives
    3. Bowel and Bladder Training
    4. Care Plans
    5. Clinical Overview
    6. Skilled Documentation
    7. Rehabilitation Documentation
    8. Weekly Summary
    9. Licensed Documentation
    10. Physician Orders
    11. Hospice
    12. MDS
    13. Change in Condition
    14. Weekly and Monthly Weights
    15. Medication and Treatment Administration Records
    16. Flow Sheets
    17. Intake and Output Monitoring
    18. Discharge
  7. Perform focused reviews of problem areas, QA concerns, Quality Indicators, trends and/or patterns, and survey issues.
  8. Maintain the admission register and complete the coding and indexing of admission diagnoses.
  9. Complete the chart closure process on all discharged resident charts.
  10. Complete the Monthly General Update Log.
  11. Complete the annual LTC-OSHPOD report.
  12. Complete the year-end census report.
  13. Maintain security of medical records.
  14. Maintain the system for record filing, retention, and destruction.
  15. Process a monthly recapitulation of physician orders.
  16. Carry out other duties as assigned by the Administrator.
  17. *Qualifications:**
  18. High school graduation or equivalent.
  19. Good physical and mental health.
  20. Knowledge of Medical Terminology.
  21. Primary computer and typing/data entry skills.
  22. General office skills, including filing and organizing.
  23. Oral and written communication skills.
  24. Good customer service and telephone skills.
  25. Skilled Nursing experience preferred.
  26. Long-term care or healthcare experience is preferable as a Coordinator of Health Information in another facility. Training as a Medical Records Secretary or equivalent.
  27. Experience with ICD-CM coding preferred.
  28. Knowledge of regulations, accreditation standards, and professional standards of practice for health information in long-term care.
  29. Knowledge of the budget process and PCC.
