So, it is our pleasure to announce that we are currently seeking afull-timecandidate for the role ofCare Coordinatorto join our Pathways to Wellness Health Home team!

By ICL definition, a Care Coordinator (CC) is a trained health professional who isresponsible for assisting and managing a patient's care. Functioning, as a member of an interdisciplinary team, C.C.s are expected to provide clinical support by providing consultation, education, interventions, and resources to maintain focus on outcomes and best practices, with the focus being to provide linkage to services to decrease high-risk medical and/or psychiatric hospitalizations.

To be considered for this role, you MUST first adhere to and understand the requirements and tasks listed below…

MINIMUM REQUIREMENTS & PREFERENCES:

  • Available Monday to Friday –Day Shift: 8:30am – 5pm
  • VALID Driver’s License Photo ID –NYS preferred
  • LOCATION: Brooklyn, NY 11207(Main Base) Various NYC Boroughs(excluding S.I.)
  • TRAVEL: up to 75% of the weekly work schedule will involve fieldwork –various modes of reliable and safe transportation use are a MUST
  • Knowledge of Medicaid, Social Security, and other entitlements –preferred
  • Knowledge of the interdisciplinary planning process and the developmental treatment model
  • Knowledge of computer software and electronic health record systems –e.g. EPIC, FCM, HCS, etc. preferred but not mandatory
  • Strong communication, interpersonal, writing, and conveying skills –bi-lingual is a plus but not mandatory
  • Proper attire at all times –this includes business casual and/or professional per the ICL dress code policy
  • Following all safety guidelines per the performance of tasks mentioned

MINIMUM EDUCATION & EXPERIENCE REQUIREMENTS:

  • OPT 1: AA Degree and 5 years of experience in EXP 1 or 2(see options below)
  • OPT 2: BA Degree and 3 years of experience in EXP 1 or 2(see options below)
  • OPT 3: MA Degree and 1 years of experience in EXP 1 or 2(see options below)
  • EDU:Acceptable Fields… social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech, and hearing, or other human services field – SOCIAL WORK or EDUCATION mainly preferred
  • EXP 1:direct clientele service – e.g. serious mental illness, developmental disabilities, or substance abuse disorders preferred; or
  • EXP 2:linking such clientele to a broad range of services essential to successful living in a community setting – e.g. medical, psychiatric, social, educational, legal, housing, financial services, etc.
  • A PLUS:Licensed in NYS as a Registered Nurse (RN), or Certified in NYS as a Credentialed Alcoholism & Substance Abuse Counselor (CASAC), or A NYS Teacher’s Certificate

TRAINING REQUIREMENTS:

  • Mandated training on the NY State Community Mental Health Assessment instrument and additional required training
  • Specific training for the designated assessment tool(s), the array of services and supports available, and the client-centered service planning process
  • Training in the assessment of individuals whose condition may trigger a need for HCBS support and ongoing knowledge of current best practices to improve health and quality of life

CARE COORDINATOR CLIENTELE TASKS:

  • Provide crisis intervention and follow-up
  • Provide phone and face-to-face outreach, engagement, and service planning in the field
  • Conduct initial and ongoing assessments of assigned clients to document strengths, needs, goals, and resources
  • Coordinate care planning with other providers of services/resources to ensure goal-directed, collaborative care, including care transitions
  • Monitor overall service delivery to clients to ensure coordination and continuity; advocate with service providers and resources as needed
  • Act as a linkage to community services including medical, behavioral, residential, entitlement, and any other needed services per the interdisciplinary care plan
  • Provide care to a variety of clients with few and/or multiple medical conditions that include but are not limited to medical comorbidities and co-occurring substance abuse disorders and so on
  • Maintain effective communication with clients, primary care physicians, substance abuse, and mental healthcare providers, family, collateral resources, and other agency staff on behalf of clients
  • Advocate and support the client's engagement with community agencies, health care providers, and others on their behalf to ensure access to services needed to help increase wellness self-management and reduce emergency room visits and/or hospitalizations

CARE COORDINATOR OFFICE TASKS:

  • Maintain effective documents, records, statistics, and other related reports in an organized, timely, and accurate manner as per policy and procedure
  • Participate in the development, documentation, review, and update of a client-centered comprehensive integrated, interdisciplinary care plan in consultation with other team members to ensure focus on desired outcomes
  • Attend and participate in team meetings to provide input and feedback around the following to review client status, update plans and goals, and review outcomes to further program goals… - psychosocial, substance abuse issues and resources, medical conditions, comorbidities, and so on
  • Perform other job-related tasks as assigned in an organized and detail-oriented manner
