Hyacinth AIDS Foundation’s mission is to help people live with HIV, stop the spread of the epidemic, and serve as a critical voice in the public debate surrounding AIDS in New Jersey. The following foundational behaviors are expected as an employee of Hyacinth:

Employees of the Agency may learn certain facts about individuals being served by the Agency that are of a highly personal and confidential nature. § Under the supervision of the Regional Services Manager, performs assigned duties relating to the implementation and evaluation of community health worker services.

Customer m Services

§ Actively seek and identify individuals or groups requiring the agency's services.

§ Assess customer’s emergency financial assistance needs, prepare appropriate individual service plans and maintain records. Assist customers in implementing the service plan by networking and referral, internally and/or to other community resources.

§ Develop a budget assessment with each customer and provide budget counseling:

§ Collect all current income documents from the customer

§ List the client's monthly bills

§ Develop EFA specific service plans with clients to coordinate long term financial and housing stability.

§ Work closely with customers seeking rental assistance to assess their ability to achieve housing retention:

§ Collect copy of current lease and address lease terms with customer

§ Communicate with landlord to advocate for extension when necessary

§ Work closely with customers seeking assistance with utility bills

§ Collect proof of address from the customer

§ Collect the utility bill and work out a plan to diminish excessive bills

§ Assist customers with Emergency Financial Assistance applications for rent and utilities.

§ Work closely with landlords and utility companies to ensure that service will not be interrupted.

§ Work collaboratively with other local service providers to develop and maintain the continuum of services available to customers.

§ Follow up on payment with regional manager or accounting department to ensure that application was processed and payment was issued to landlord and/or gas/electric companies.

§ Monitor program expense regularly to remain consistent with disbursement expectations.

§ Develop and maintain database of professional contacts.

§ Establish, maintain and update customer case records using approved agency forms and procedures. Ensure confidentiality of all records.

§ Provide relevant literature to customers and members of the community.

§ Prepare monthly reports in accordance with agency policies and procedures and grant requirements

§ Connect customers to essential support services such as housing, food and transportation

§ Refer customers to housing and budgeting workshops

§ Maintain and Update customer information in eCOMPAS and CAREWare

· Establish, maintain, and update case records using approved agency forms and procedures

Hyacinth Representation / Advocacy

· Attend and participate in staff meetings and supervisory conferences

· Maintain working relationships with other community providers

· Provide written reports and maintain statistics as required

· Represent the agency at community events, meetings, and conferences

· Adhere to Hyacinth policies and procedures including customer confidentiality provisions

· Proficiency in written and verbal communications

· Proficient computer skills including Excel, Word, Outlook
