Care manager will possess a bachelor's degree in social work, psychology, gerontology or other related field or be a registerd nurse and one year of experience in direct service to older persons. Responsible for provision of care management services to older consumers. Can include in-home assessments, developing care plans, coordinating services with other agencies, evaluating effectiveness of services with consumer's needs. Entering information into state-approved computer data base. Complete home visits. Work with guardianship program. Educate and assist older consumers with obtaining services. Counsel consumers and families. Assist protective service workers with investigation referrals and have general knowledge of protective service regulations and basic social casework principles. Assist with in-office I&R and telephones. Excellent verbal communication, organization and computer skills.
