Hudson Valley Hospice is Hiring an Admissions RN - Team Lead Near Poughkeepsie, NY

Description

The overall responsibility of The Admissions RN - Team Lead is the clinical supervision, coordination, and management of the Admissions RNs team. Responsible for the daily operations planning and priority setting for patient evaluation, admissions, and oversight, management, and evaluation of staff. The Admissions RN Team Lead works closely with the Director of Admissions to coordinate schedules, answer concerns and questions and provides guidance to the other RNs (Registered Nurses). We are looking for someone who is thoughtful in their decision making process, can act quickly with any employee concerns/issues, and can showcase a level of managerial skill.

ESSENTIAL FUNCTIONS

  • Responsible for the coordination of the Referral Center admission schedule to ensure availability of RNs to admit patients in a timely manner 7 days a week.
  • Completes employee field evaluations that are consistent with job expectation, employee communication and completed within an appropriate time period
  • Responsible for making joint visits, when requested, for orientation, preceptor, and mentorship of admission RNs
  • Provides clinical expertise and support to full-time, part-time and per diem Admission RNs with daily activities that arise with regard to patient issues.
  • Works with Director of Admissions to ensure communication regarding open referrals and coordination of admissions in a timely manner.
  • As required, may function as an Admission RN
  • Participates in Quality Assessment/Performance Improvement Program as appropriate; actively contributes to PI data collection and process utilizing the PDCA model.

WORK ENVIRONMENT/PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Works in assigned settings, patient/family homes and community agencies.
  • Moderate to high risk of exposure to unsafe environments.
  • Ability to carry up to twenty-five pounds of supplies and/or materials used to provide community education.
  • Moderate amount of time sitting and working with a computer screen

PREFERRED EDUCATION & EXPERIENCE

  • Current registration as a registered nurse in New York State. Certified as a Hospice Palliative Care Nurse or obtain certification after one year of employment.
  • BS or BSN (currently attending college to obtain a BSN) or commensurate experience, clinically and/or managerially.
  • Minimum of three years of clinical nursing, preferably in oncology, critical care and/or home care; at least one year of which is medical-surgical nursing.
  • Valid NYS Driver’s License
  • Managerial/Leadership experience preferred.

Hours: Rotating Schedule of (4) 10-hour shifts, 8:30am-6:30pm: weekend and holiday flexibility when needed
Pay: $90,000-100,000 per year; depending on experience
FLSA: Exempt
