The RN Case Manager for the Nursing Home team will provide case management and nursing services to Hudson Valley Hospice patients and their families and facilitate relationships with nursing facilities ensuring that services are consistent with the philosophy, goals, objectives, and policies of the Agency.

ESSENTIAL FUNCTIONS

• Follows the appropriate standards of practice established by Hudson Valley Hospice and the American Nursing Association and National Hospice Organization.
• Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural, and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
• Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
• Update admission care plan within 5 days of admission.
• Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
• Update the care plan a minimum of every 14 days.
• Document and computer entry of all interventions of phone and SNF assessment visits within 24 hours of the intervention or notification thereof.

WORK ENVIRONMENT/PHYSICAL DEMANDS

• Work in various nursing facilities environments.
• Ability to lift at least 25 pounds
• Moderate/High level of possible exposure to infectious agents.
• Average amount of travel by car.
• Moderate to high risk of exposure to unsafe environments.
• Routine duties involve potential for direct exposure to blood or body fluids.

PREFERRED EDUCATION & EXPERIENCE

• Graduate of an accredited nursing school.
• Some nursing experience preferred - willing to train New RN Grad
• Valid New York State nursing license.
• Valid New York State drivers license
