Case Manager I - PACE Program - (Winkler Office)

Ability to complete and submit Medicaid applications on time for Medicaid pending clients. Work to regain Medicaid coverage with clients who lose their Medicaid. Collaborates with PACE IDT related to client needs or questions related to plan of care. Works closely with State Dept. of Elder Affairs caseworker related to client eligibility. Bachelor’s degree in a social work, health-related field or licensure as a health professional; Must be willing to travel across Lee, Collier and Charlotte counties.
