Your primary focus will be to support individuals and families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life. • Perform and coordinate field visits with the nursing team and as needed by the patient plan of care and in an effort to support their family
• Train and educate patient, family/caregivers and staff on appropriate usage of medications, DME, and supplies as well as the disease progression
• Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention
• Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language that the patient and family understand
• Demonstrate excellent interpersonal and communication skills.

• Degree in Nursing (ADN or BSN)
• Valid RN Texas License
• At least 1-year Hospice experience (preferred)
• Valid Texas Driver's License and Proof of Auto Insurance
• Reliable Transportation
• Valid BLS
• Successful completion of Background Screen including employment verification
