Weekend On-Call LVN/RN - Hospice

Weekend on-call hourly rate, per visit rate, and mileage will be compensated.
Responsibilities:
- The weekend schedule begins at 5 pm on Friday and ends at 8:30 am Monday morning.
- Perform on-call PRN visits.
- Perform and coordinate field visits with the nursing team as needed by the patient's plan of care and in an effort to support their family.
- Train and educate patients, family/caregivers and staff on appropriate usage of medications, DME, and supplies as well as the disease progression.
- Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention.
- Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language the patient and family understand.
- Demonstrate excellent interpersonal and communication skills.
- Demonstrate Hope Health Care's agency values - compassion, dignity, integrity, professionalism, collaboration, balance, growth and ethical behavior.
Qualifications:
- Degree in Nursing.
- Valid LVN or RN Texas License.
- At least 1 year of Hospice experience (preferred).
- Valid Texas Driver's License and Proof of Auto Insurance.
- Reliable Transportation.
- Valid BLS.
- Successful completion of Background screening including employment verification.
Compensation:
- Weekend on-call hourly rate, per visit rate, and mileage will be compensated.
