Your primary focus will be to support individuals and families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life. • Perform and coordinate field visits with the nursing team as needed by the patient's plan of care and in an effort to support their family.
• Train and educate patients, family/caregivers and staff on appropriate usage of medications, DME, and supplies as well as the disease progression.
• Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention.
• Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language the patient and family understand.
• Demonstrate excellent interpersonal and communication skills.
• Demonstrate Hope Health Care's agency values - compassion, dignity, integrity, professionalism, collaboration, balance, growth and ethical behavior.

• Degree in Nursing.
• Valid LVN or RN Texas License.
• At least 1 year of Hospice experience (preferred).
• Valid Texas Driver's License and Proof of Auto Insurance.
• Reliable Transportation.
• Valid BLS.
• Successful completion of Background screening including employment verification.
