This job will be based in Brooklyn, NY.
Intake Specialist must be fluent in Spanish.
Responsibilities:
· Creating and managing patient profiles in our CRM.
· Collecting all necessary documents to complete referral process and updating CRM on status.
· Determining if the agency can safely service patient and referring pt proper long term or short-term care agency based on current insurance information.
· Ensure insurance information is accurate, verified and entered into CRM.
· Educating patients on alternate insurance plans as needed to obtain long term home care services.
· Ensure Intake Status is updated on a weekly basis.
· Responding to all potential new cases in a timely manner.
· Enter data for all new cases into appropriate places.
· Updating HHA exchange when authorizations from payors change and notify team.
· Act as liaison between patient and long term or short-term care program.
· Communicate with referral sources regarding staffing availability.
· All other duties assigned by supervisor.
· Complies with HIPAA code of conduct and regulations.
Requirements:
· Excellent communication abilities, including speaking, writing and active listening.
· English speaking.
· Bilingual Spanish preferred.
· Great customer service skills, including a personable and positive attitude.
· High typing speed and accuracy.
· Good computer skills, including basic troubleshooting and using word processing, spreadsheet, presentation and email software applications.
· Problem-solving, critical thinking and decision-making abilities.
· Ability to work independently with little-to-no supervision.
· Keen attention to detail.
· Team attitude.
