Home Start is Hiring a CalOES Case Manager Near San Diego, CA

Position: Transitional Housing Case Manager

Classification: Non-Exempt

Supervised By: Program Manager

Job Summary: Provides case management services and support to short-term and transitional housing clients in the XH program. 

Duties and Responsibilities:

  1. Coordinate with referral sources and respond within 48 working hours;
  2. Provide mobile advocacy services and support to victims of crime;
  3. Meet with survivor and complete intake and assessment;
  4. Adhere to Home Start principles and practices to include Trauma Informed Care; Harm Reduction and Safety Planning; Mobile Advocacy and Flexible Engagement. 
  5. Provide coordinated entry and intake assessments for participants seeking housing support, identifying housing service needs and coordinating initial services;
  6. Develop individualized plans for delivery of housing and supportive services that are tailored, survivor driven, and trauma informed, and which include short- and long-term goals for safety and self-sufficiency;
  7. Help participants establish linkages to community services and resources that help them achieve their goals;
  8. Work with the survivor in an advocacy capacity helping them understand and navigate through various systems including child welfare, social services, and juvenile justice (i.e. sealing juvenile delinquency and/or adult criminal records);
  9. Help survivors establish and strengthen their support network of peers, family and community members as well as to self-help groups;
  10. Encourage participants to make choices that reduce risk and promote safety;
  11. Assist qualified survivors complete paperwork for the California Victim Compensation Program;
  12. Complete and maintain client records that document services provided;
  13. Facilitate access to flexible financial assistance to address barriers to safety and stability, including assistance with housing and non-housing needs such as transportation (i.e. car repair), childcare, utility bills, school supplies, uniforms and permits required for employment;
  14. Complete and maintain client records that document services provided;
  15. Engage in ongoing safety planning;
  16. Provide follow-up services to XH clients for at least 6 months after they leave the program;
  17. Complete all required training to include DV Counselor training (40 hours) and 24 hours of pre and in-service training on topics such as Trauma Informed Care, Cultural Competency, Motivational Interviewing and Positive Youth Development, Client Confidentiality/HIPPA, etc.;
  18. Participate in system data collection; complete necessary paper work and forms. 
  19. Other duties as assigned. 

Qualifications:

Education A Bachelor’s Degree in human services or related field or equivalent work or volunteer experience and training preferred. Skills: Must work well with persons of varied sexual orientation, racial, ethnic, cultural, socioeconomic backgrounds, and with individuals who are differently-abled. Awareness of relationship violence, child abuse, stalking, and sexual abuse and assault preferred. Knowledge of counseling and advocacy techniques for people who have been victims of crime, including crisis intervention and case management skills beneficial. Previous employment or volunteer work in human services, community agencies or other related non-profit or social change organizations a plus. Good oral and written communication and computer skills are required. Bilingual (Spanish/English) speaker preferred. Other Requirements: Must be a Certified DV Counselor (completed 40-hour DV Counselor training pursuant to Evidence Code §1037.1) or willing to complete such training within 3 months of hire. Must pass a criminal background check. Must have a valid California driver’s license, a safe driving record, automobile insurance, and a reliable vehicle.

Physical Demands:

  • Reading and typing on a computer frequently.
  • Sitting for long periods of time at required meetings or in supervision.
  • Daily traveling in car to meet with clients and attend agency and community meetings.

Work Environment:

  • Office environment. 
  • Travel to meet with clients.
