POSITION TITLE: Home Study Case Manager

PROGRAM: Safe Passages

DEPARTMENT: Refugee Services

REPORTS TO: Department Director

SALARY: $25-27 per hour

FLSA STATUS: Non-Exempt

POSITION DESCRIPTION

The HS Case Manager, under the supervision of the Department Director, provides case management and supportive services to unaccompanied immigrant children released from federal custody. The HS Case Manager will work in collaboration with program staff to cultivate a client-centered and trauma-informed approach to service provision. The HSCM is responsible for conducting home studies to unaccompanied children reuniting with sponsors. The HS Case Manager assesses household needs, coordinates resources based on goals and provides advocacy, information and referral services.

JOB RESPONSIBILITIES

  • Provide ongoing case management focusing on screening and making appropriate referrals to address the minor’s needs related to placement and safety issues, education, physical and mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice and child enrichment activities
  • Conduct home visits and make phone contacts to assess potential risks of placement & determine whether the sponsor/caregiver and proposed environment are suitable
  • Provide sponsors with psychosocial education on the minor’s individual needs
  • Identify family strengths and social support
  • Help families develop individualized plan for successful integration
  • Complete and document required reports, case notes and contacts in the appropriate database in a timely manner
  • Collaborate with law enforcement agencies, shelter clinicians to ensure the safety and well-being of the minor
  • Provide crisis intervention services directly, as needed, or connect family to services to ensure minor’s safety and well-being in a crisis situation
  • Actively establish, enhance and maintain liaison relationships with key government agencies and non-profits/community organizations
  • Comply with policies and procedures governing the program and ensure that the program is operating at maximum efficiency
  • Meet or exceed program productivity expectations on a regular or sustained basis
  • Proficiency in Spanish/English, written and spoken
  • Knowledge of community resources and ability to connect families to community services
  • Ability to work independently in the field as well as collaboratively as a team
  • Organized and flexible with a high degree of integrity, reliability, initiative and adaptability
  • Ability to handle and maintain confidential information per HIPAA protocols
  • Assist in other special projects for Safe Passages as needed

QUALIFICATIONS:

  • Demonstrates clinical/technical judgment and leadership skills and ability to multitask
  • Experience in using strengths-based approaches with clients
  • Excellent written and verbal skills
  • Understanding and applying cultural humility
  • Excellent interpersonal skills
  • Contributes knowledge and understanding to the agency staff in areas of specialized competence
  • Applicants must have the ability to work successfully with a broad range of stakeholders, including clients, staff, volunteers and the community
  • Must complete background checks (CA/N checks; FBI fingerprints)

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Must possess a valid driver’s license, have a good driving record, and have at least three years of driving experience

EDUCATION and EXPERIENCE

  • Bachelor’s degree in Social Work, behavioral sciences, human services or social services fields a plus
  • Minimum of 1 year experience relevant to the position and working with vulnerable, immigrant or migrant populations
  • Child welfare and/or case management experience preferred
  • Experience in conflict management, accountability and confrontation is a must
  • Experience utilizing various Microsoft Office software products, including Outlook, Word, & Excel
  • Excellent verbal, written and listening communication with the clients and family reunification team

PHYSICAL DEMANDS:

  • Sedentary work that primarily involves sitting/standing
  • Remaining in a stationary position, often standing or sitting for prolonged periods
  • Repeating motions that may include the wrists, hands and/or fingers
