Compensation: $16 – $17.50 per hour

Our team ofPatient Care Coordinators are instrumental in providing a seamless patientexperience and making sure all documentation and information is collected andorganized appropriately. Successful candidates will have a passion forproviding a high level of customer service to home care patients. Additionally,selected candidates will have had prior customer service experience in afast-paced environment.
Patient Care Coordinator Responsibilities:
•Send and track required face-to-facedocumentation for Physicians to sign and return
•Collect information for currently hospitalizedclients: H&P (History & Physical), ROC (Resumption of Care), MD orders,etc.
•Conduct pre-admit calls to verify clientinformation, as well as five day and discharge follow-up calls
•Track hospitalized clients and notify CommunityLiaisons of discharges, in order to resume providing care
Patient Care Coordinator Qualifications:
• High school graduate; college degree preferred
• Knowledge of medical terminology is strongly preferred
• Minimum of one (1) year of experience in an office or health care settingpreferred
• Strong verbal, written, and telephonic communication skills
• Demonstrated ability to type at least 50 words per minute
