The Regional Training Coordinator will provide training and support to HHUNYs central and southern New York region Health Home network of Care Management Agencies. The Regional Training Coordinator will work in collaboration with the HHUNY Training Team as well as the other Regional Training Coordinator to develop content and provide trainings. Training programs developed and implemented will support delivery of person-centered, recovery-focused care management in a manner consistent with New York State requirements, HHUNY policies/practices/procedures, and Netsmart CareManager as the Health Information Technology (HIT) tool.
This position provides in-person and web-based trainings, as well as coaching, mentoring and team-based learning to sustain change in practice and associated improvement in care management outcomes. This position additionally provides training for community-based organizations and conferences.
This position requires the employee to be located in upstate New York, preferably within the Central or Southern region of NYS.
Minimum Qualifications:
Bachelors degree in related field, or equivalent education and work experience.
Demonstrated experience in human services arena, including training and new program development and implementation. Experience in care management practice training and implementation is a plus.
Experience working within adult Health Home services and Netsmart CareManager is preferred.
