Healthy MD is Hiring a Lead Case Manager Near Tallahassee, FL
In charge of clinic inventory and ordering supplies for the team
Be the liaison between the DOH and clinic staff
Participate and lead outreach events
Establish community partnerships/affiliate sites
Track all clinical certifications for the entire clinic staff (eg. 500/501, Oraquick, phlebotomy, etc.)
Serve as the on the job/field trainer for all new staff members
Take over the management of all Hep C patients
Reports incidents /employee relations concerns to site manager
Perform regular case manager duties
Any other responsibilities and task assigned my management
Minimum Requirements:
High School Diploma or Equivalent
Knowledge of Windows Office Systems; particularly Microsoft Word, Excel, PowerPoint, and Outlook
Strong analytical and problem-solving skills
Strong work ethic; excellent communication and interpersonal skills
Has the ability to interact effectively with clients, administration, and staff
Requires the ability to communicate with clients as well as target groups of diverse individuals
Preferred Requirements:
Experience with Cover My Med platform
Infectious Disease knowledge (HIV/STI/HEP-C)
Medical terminology
Knowledge of EMR/EHR systems)
Highly organized and a proven strategic thinker with ability to engage and motivate teams
