Job Overview: We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals in need, helping them navigate through various challenges and connect with necessary resources. This is a rewarding opportunity to make a positive impact on the lives of others. Duties: - Conduct comprehensive assessments to identify clients' needs and develop individualized care plans - Provide crisis intervention and support to clients experiencing behavioral health issues - Collaborate with clients to set goals and develop strategies for achieving them - Assist clients in accessing appropriate social services, such as housing assistance (Section 8), workforce development programs, and addiction counseling - Coordinate with other professionals, including social workers and healthcare providers, to ensure holistic care for clients - Utilize motivational interviewing techniques to empower clients and promote positive change - Monitor client progress and adjust care plans as needed - Maintain accurate and up-to-date documentation of client interactions and services provided Experience: - Bachelor's degree in social work or related field (Master's degree preferred) - Proven experience in case management or related roles - Knowledge of behavioral health principles and crisis management techniques - Familiarity with community resources and social service programs - Strong communication skills, both verbal and written - Ability to work collaboratively in a multidisciplinary team - Excellent organizational skills and attention to detail If you are passionate about helping others, possess the necessary skills and qualifications, and are looking for a fulfilling career as a Case Manager, we encourage you to apply.
