Education Requirements: *1 of the following required*
Bachelors or Masters in Social Work, Psychology, Sociology, or Nursing.
Certified Medical Assistant with an Associate Degree
Licensed ARNP
Licensed Agency Affiliated Counselor
Licensed Clinical Psychologist
Licensed Marriage and Family Therapist
Licensed Mental Health Counselor
Licensed Physician's Assistant
Licensed Psychiatric Nurse
Licensed Psychiatrist
Licensed Registered Nurse
Licensed Practical Nurse
Experience Preferred:
Minimum one year of experience working with individuals who experience a developmental or intellectual disability
Developmental Disabilities Include:
● Intellectual Disability
● Cerebral Palsy
● Epilepsy
● Autism
● Another neurological or other condition similar to Intellectual Disability (acquired before age 18)
Job Responsibilities:
● Conduct initial client outreach and scheduling of Care Coordinator visits to introduce Health Home benefits.
● Perform comprehensive health assessments, collaborating with interdisciplinary teams and primary care physicians.
● Develop, revise, and implement Health Action Plans (HAP) with clients and their support network.
● Coordinate with service providers to secure necessary care and crisis intervention.
● Link clients to required services and educational resources, promoting community participation and health literacy.
● Facilitate cross-system care coordination and follow-up activities post-discharge.
● Advocate for client needs, assist with appointment scheduling, and provide educational materials.
● Support clients in accessing community supports and entitlement programs.
● Provide information on upcoming community events and assist with health care service applications.
