Through its core values of innovation, engagement, and health equity, HCPH strives to bring meaningful solutions to public health issues while keeping Harris County healthy and vibrant.Position Overview:The Behavioral Health Counselor (BHC) is responsible for providing emotional and mental health support to Harris County residents utilizing the Sheldon HUB care team initiative. The BHS will provide behavioral health services such as mental health and substance-use community assessment and referrals as well as Social Determinants of Health (SDOH) assessment and community resource navigation. This program intends to expand comprehensive mental health resource navigation by providing access to behavioral health specialists competent in the field of mental health, therapeutic counseling and addiction/substance-use prevention resources. The Behavioral Health Counselor is accountable for ensuring delivery of exceptional services that are focused on the client's needs. 
Duties & Responsibilities:
  • Assess for participant's behavioral health (mental, emotional and substance-related) needs by completing biopsychosocial evaluations and SDOH assessments to address non-medical drivers to participants total health profiles
  • Engage with participants in solution-focused evidence based counseling styles that guide clients to self-reflection and self-determination in brief counseling interactions
  • Maintain a collaborative care model framework in case management of participants including but not limited to healthcare providers by engaging their additional HUB providers/internal HCPH programs, as it relates to progress towards care plan goals
  • Initiate referrals to community behavioral health resources, and maintain follow-up on high-risk individuals/families
  • Evaluate and record clients’ overall program progress, including assessments, progress notes, referrals and all follow-up interactions in the EMR. Maintain weekly scheduled appointments in the electronic medical records (EMR) system
  • Build and maintain trusted relationships with program participants, partners, stakeholders, and administrators. Conducts outreach activities to inform community groups/agencies about HCPH Behavioral Health Services
  • Other duties as assigned including special tasks related to responding to emergency events

Education:
  • Master's degree in Social Work, Psychology, Public Health, Health Education, or related field
  • Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC), or Licensed Family and Marriage Therapist (LMFT)
  • Valid TX Driver’s License (upon hire)
Experience:
  • Two (2) years' experience providing direct mental health services OR one (1) year providing services to high-risk vulnerable populations
Knowledge, Skills and Abilities:
  • Maintain necessary licensure and certifications required to practice in the State of Texas.
  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)
  • Excellent communication skills (verbal & written)
  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
  • Ability to maintain confidentiality when dealing with sensitive information
  • Ability to work well with public and staff members

  • Full-Time | Regular
  • Monday through Friday, from 8am to 5 pm.
  • Must be willing to flex work hours when required, including weekends.
  • Able to travel within Harris County as needed.
Work Environment:
  • This job will operate in a community clinic setting located at CE King Middle School collaborating with Sheldon community partners.
  • This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. 
Physical Demands:
  • This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.

  • Training may take place at different clinics within HCPH Jurisdiction 
Employment is contingent upon passing a background check and drug screen.
