Through its core values of innovation, engagement, and health equity, HCPH strives to bring meaningful solutions to public health issues while keeping Harris County healthy and vibrant.Position Overview:Under the supervision of the Reentry Cohort Manager, the Social Worker (SW) is responsible for supporting ACCESS enrolled participants by providing community resources and direct mental health counseling if needed. This position will facilitates the collaborative interdisciplinary process of social work encompassing assessment, planning, facilitation, care coordination, and evaluation appropriate to the social work scope of licensure. The SW helps participants address and resolve their comprehensive social, financial, and psychological needs. The position will also ensure compliance with standards established by professional organizations; national, state, and local regulatory agencies; and institutional policies and procedures. The SW is accountable for practice that upholds high standards of integrity and ethical behavior.This is a grant funded position.Duties and Responsibilities:• Develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, and participant therapy. Uses clinical knowledge and screening tools to identify need for social work intervention• Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence and of mental health and/or substance abuse problems in patients and family members• Plans and organizes caseload to reflect priorities based on care coordination plans, patient needs, patient goals, policies, and operating procedures• Maintains productivity and caseload expectations set by the ACCESS Department• Completes a full assessment based on the social work assessment, identifying social determinants of health. Completes/reviews appropriate ACCESS screenings/assessments and/or virtual client record• Attends Care Coordination Conferences; ACCESS participant documentation is timely, concise, relevant, complete and grammatically correct• Facilitate individual and group therapy sessions as needed with program participants of various cohorts• Adheres to cultural competency standards and provides every participant with equitable treatment and service.• Other duties as assigned, including special tasks involved in responding to an emergency event
